چکیده

Typical atrial flutter is a common reentrant atrial arrhythmia. The critical portion of the reentrant circuit is the rim of tissue between the tricuspid valve (TV) annulus and the inferior vena cava (IVC) (Fig.1).1,2 The reentrant circuit can operate in either clockwise or counterclockwise directions (Figs. 2–3). Elimination of conduction across the TV-IVC isthmus eliminates the tachycardia.3 The endpoint of ablation is to achieve bidirectional TVIVC isthmus block. Successful ablation of TV-IVC

Successful Ablation of Cavotricuspid Isthmus-dependent Atrial Flutter Guided by Contact Force Vector in a Patient After a Tricuspid Valve Replacement

A 46-year-old man after a tricuspid valve replacement due to traumatic severe tricuspid regurgitation developed cavotricuspid isthmus-dependent counterclockwise atrial flutter. During a linear ablation using a contact force-sensing irrigated ablation catheter, the flutter could be terminated by a radiofrequency application within a deep pouch just below the bioprosthetic tricuspid valve.

Electrogram polarity and cavotricuspid isthmus block during ablation of typical atrial flutter.

INTRODUCTION The atrial activation sequence around the tricuspid annulus has been used to assess whether complete block has been achieved across the cavotricuspid isthmus during radiofrequency ablation of typical atrial flutter. However, sometimes the atrial activation sequence does not clearly establish the presence or absence of complete block. The purpose of this study was to determine wheth...

Organised atrial fibrillation simulating atrial flutter.

We present a case of drug-refractory paroxysmal atrial fibrillation in which episodes of AF spontaneously alternated with a regular arrhythmia resembling atrial flutter. Isthmus-dependent atrial flutter was initially diagnosed and the patient was treated with cavotricuspid isthmus ablation. Two months later the patient returned with recurrent arrhythmias. On the 12-lead ECG there was again evid...

Incisional Atrial Tachycardia Masquerading As Counter-clockwise Atrial Flutter

Forty-four-year-old male patient with surgical closure of ostium secundum type atrial septal defect at the age of 9 years had undergone radiofrequency catheter ablation of the cavotricuspid isthmus because of recurrent drug-resistant typical isthmus-dependent atrial flutter, with bidirectional isthmus block achieved. Only few days later he started to suffer again high-rate tachycardia. An atypi...

Ablation Of Atrial Flutter:Block (Isthmus Conduction) Or Not A Block, That Is The Question?

It is important to identify residual slow conduction and minimize the chance of resumption of conduction after right atrial isthmus ablation to reduce the chance of recurrence of atrial flutter (AFL). The aim of this article is to discuss the best possible way of confirming a bi-directional isthmus conduction (BIC) block after ablation of an isthmus-dependent AFL. A combination of activation an...
